Specialized Care for Your Individual Needs
VNA caregivers provide a multidisciplinary COPD program that may include:
- Skilled nursing care
- Physical and occupational therapies
- Home health aides
- Telehealth monitoring
- COPD workbook
Telehealth monitoring is a small monitor provided in the home that:
- Daily measures your heart rate, blood pressure, and other important indicators
- Asks and records a series of daily questions for a comprehensive assessment of patient status
- Is monitored by experienced clinical staff
Each patient who has a diagnosis of COPD receives specialized COPD workbook and teaching from clinical staff.

A Teaching Approach to Patient Care
Your education plays an important role in managing your disease and the decision-making process about your treatment.
The VNA nurse and treatment team will assist you and your family with:
- Recognizing signs and symptoms of worsening of disease
- Providing on-call nurse 24 hours a day, 7 days a week, to assist with complications when on VNA service
- Advising when additional services might be needed
