Specialized Care for Your Individual Needs

VNA Diabetes caregivers enable patients like you to independently manage your disease at home, using specially-developed American Diabetes Association (ADA) standards of care.

VNA Diabetes Care is a multidisciplinary approach that includes supervision by a Certified Diabetes Educator. Care is administered through professional:

  • Registered nurses
  • Physical and/or occupational therapists
  • Medical social workers

Focus of treatment is on:

  • Effective disease and diet management
  • Comprehensive, education-based service
  • Psychological aspects of disease
  • Prevention

Philosophy of VNA Diabetes Care Program

VNA is committed to providing a quality diabetes care program to:

  • Assist you in effectively managing a chronic condition for which there is no cure
  • Listen, respond to, and truly care for improving your quality of life
  • Help you regain health and functional independence, empowering you to remain in your home
  • Provide you with a glucometer to facilitate blood sugar monitoring for short- and long-term care

A Teaching Approach to Patient Care

Timely and effective communication with patients, families, doctors, social workers, and others involved in your care is the cornerstone of VNA’s service concept.

Nurses utilize a Disease Workbook to teach you the ADA Standards of Diabetes Care, including:

  • Lifestyle management
  • Proper drawing techniques for blood sugar analysis and insulin administration
  • Medical nutrition therapy
  • Physical activity
  • Hypoglycemia/Hyperglycemia

The VNA nurse and treatment team will assist you and your family with:

  • Recognizing signs and symptoms of worsening of disease
  • Providing on-call nurse 24 hours a day, 7 days a week, to assist with complications when on VNA service
  • Advising when additional services might be needed
