Specialized Care for Your Individual Needs
VNA Bridge Care
The VNA Bridge Care service allows a terminally ill patient with limited life expectancy, who is not yet ready to embrace hospice care, to receive quality, transitional home healthcare. The VNA “care team” works closely with the physician to provide the best quality of life for the patient, including:
- Assisting patient and family if not ready for hospice
- Providing equipment and supplies at no cost to patient/family
- Using hospice-trained staff to assist with pain and symptom management
- Transitioning from bridge to hospice-level care when patient and family need additional services
VNA Hospice
VNA’s Hospice provides everything necessary when a patient needs a special kind of care, including:
- Medications (prescription and over-the-counter) related to disease/terminal illness provided at no charge to patient/family
- All equipment and supplies related to terminal illness provided at no cost to patient/family
- On-call, skilled nursing to assist 24 hours a day, 7 days a week for triage of crises
- Affiliated with hospitals and nursing homes
- Bereavement counseling up to 13 months for loved ones or anyone identified
- Volunteers specifically hospice-trained to assist
- An interdisciplinary team that includes a:
- Medical Director
- Case Manager
- Home Health Aid
- Social Worker
- Chaplin
- Volunteer
- Coordinator
VNA Hospice Provides Four Levels of Care
- Home Hospice – Provides pain management and attends to the emotional needs of the patient, allowing the patient to stay in the comfort of their home
- Respite Care – Caregiver benefit of up to 5 days per certification
- Continuous Care (crisis care) – Provided under Medicare hospice benefit for up to 24-48 hours of skilled care for acute pain/symptom management, until of a level of control is obtained
- In-patient Care – Provided under Medicare hospice benefit for short stay due to pain/symptom management and resulting better continuity of care for patient and family
Patient Eligibility
- Patient must have a terminal diagnosis and a prognosis of six months or less if disease runs to normal course as suggested/recommended by a physician
- Patient/family agree to palliative, not corrective care
- Patient terminal diagnosis must be recertified periodically by the treatment team and a physician along with family and patient
