Specialized Care for Your Individual Needs

Based on the physician’s orders, a VNA “care team” is assembled in which members work in a coordinated plan, each working with you, the patient:

  • Physical Therapist – Fall prevention, posture, strengthening exercises, gait training, etc.
  • Occupational Therapist – Handwriting, daily activities (dressing, bathing, fine motor coordination)
  • Speech Therapist – Swallowing, voice volume, and projection
  • Medical Social Workers – Assist you and your caregiver on relationship, stress management, and other disease-adjustment issues
  • Nurse – Medication management, compliance, and disease management and diet education

Philosophy of VNA’s Parkinson’s Disease Care Program

The comprehensive care provided by VNA is designed to help you:

  • Maintain the highest quality of life possible
  • Regain mobility, speech, and language skills
  • Remain in the home for as long as safely possible
  • Obtain appropriate adaptive equipment

VNA’s goal is to help your family manage the chronic disease by:

  • Effectively adapting to a lifestyle with the disease
  • Early detection of complications, and recognizing progression of disease
  • Educating your caregiver(s) about community services to assist them in dealing with the disease to avoid burnout

A Teaching Approach to Patient Care

Your education plays an important role in managing your disease and the decision-making process about your treatment. VNA will:

  • Help you and your caregiver recognize signs and symptoms of worsening of disease
  • Aid in diet and nutrition management
  • Educate on medication management

The VNA nurse and treatment team will assist you and your family with:

  • Providing on-call nurse 24 hours a day, 7 days a week, to assist with complications when on VNA service
  • Advising when additional services might be needed
