VNA Long Term Care
For most of us, injury and illness represent short-lived challenges. But for those with acute or chronic illness or disability, life-time care may be necessary.
VNA Long Term Care is an alternative to institutional care for persons with chronic illnesses or disabilities. Community Based Alternatives, Primary Home Care and other VNA programs help keep families intact through home-based support services.
VNA is a STAR-PLUS network provider. VNA provides service for the
following HMO's through this program: Dallas Service Area—Molina Texas
Community Plus and Superior Health Plan Plus; Tarrant Service Area—
Amerigroup Community Care and Bravo Health Texas, Inc.
